Practice Policies

Medication Refills

Please submit all requests for refill through the patient portal at least 5 business days before your refill is due (this allows time to address medication shortages or insurance rejections). Refill requests via email and voicemail will not be honored.

Please inform us of any need for prior authorization or refill request. DO NOT ASSUME THE PHARMACY HAS INFORMED US.

You must be seen every 90 days by the practice. If you have not been seen within 90 days the practice will not provide a refill of medication until you have been seen.

Controlled Substances

Little Meadow Psychiatry requires that all patient’s receiving a controlled substance complete a urinalysis toxicology screen at time of intake and any time thereafter at the discretion of the prescriber.

Per state and federal regulations, controlled substances cannot have refills on file. You are responsible for being seen on a routine basis in order to receive your refill.

Any patient found to be requesting controlled substances from multiple providers will be discharged from the practice immediately.

Appointment Policies

24 hour advance notice is required if you need to reschedule.  Otherwise, there will be a $100 late cancellation fee.

If you fail to attend your appointment, you will owe the full out of pocket fee for the missed service ($165 for a medication management follow-up and $350 for an intake appointment).

Little Meadow Psychiatry allows for a 10 minute grace period for you to log-in to your appointment before the appointment is marked as a no-show. If you are experiencing technological issues, please contact your provider within the first 10 minutes of your scheduled appointment time.

3 missed appointments will result in discharge from the practice.

Financial Policies

Your fee for service (co-pay or out of pocket fee) will be charged at the time of the appointment.

If you have an unpaid balance greater than 30 days old your follow up may be canceled until the balance is paid.

Communication

All patient communication and telehealth visits will take place through IntakeQ’s web portal.  This is a secure and HIPPA compliant platform.  IntakeQ will keep a record of all communications for patient and provider reference.

Communication will ONLY occur via the IntakeQ portal.

All medication adjustments and concerns must be addressed in an appointment. Medication adjustments will not be made via portal communication.

All communication will be addressed at end of business day (please note that communication on Wednesdays and Thursdays will be addressed on Friday).

Little Meadow Psychiatry does not provide after hours or weekend coverage.  If you are experiencing a psychiatric or medical emergency please seek emergent care by calling 911 or 211 for mobile crisis, or going to the nearest emergency department.
